It has been used in medicine for decades, valued particularly as a relatively safe anaesthetic, and it also has more recent and important uses in treating severe depression and in pain management. At the same time, ketamine is misused recreationally as a dissociative drug, sometimes known by names such as Special K, for its mind-altering effects. So ketamine has a dual identity: a valuable medicine on the one hand, and a misused drug with real risks on the other.

Medical uses of ketamine

In these roles, ketamine is a well-established and useful medicine, used by trained medical professionals in controlled settings.

A newer and increasingly important medical use of ketamine is in the treatment of severe, treatment-resistant depression, that is, depression that has not responded to other treatments. Ketamine, and a related medication called esketamine given as a nasal spray, can produce rapid improvements in depression, sometimes within hours or days, which is much faster than traditional antidepressants, and can be helpful in serious cases, including where there are urgent concerns such as suicidal thoughts. This use is carried out in carefully controlled medical settings, under supervision, with specific protocols, and is quite different from recreational use. It represents a significant development in psychiatry. However, this medical use is distinct from, and should not be confused with, recreational ketamine use, which carries serious risks and lacks the controls and supervision of medical treatment.

Recreational use and effects

Ketamine is also widely misused recreationally, often in club, party, and festival settings, where it is taken, usually as a powder that is snorted, for its dissociative, hallucinogenic, and euphoric effects. Recreational users seek the altered state ketamine produces: a sense of detachment, dreamlike and distorted perceptions, euphoria, and, at higher doses, the profound dissociation of the k-hole, an intense, immersive, out-of-body or trance-like state of near-total separation from reality. The effects come on relatively quickly and are usually shorter-lived than those of some other drugs. Some people find these effects pleasurable or fascinating, while others find them disorienting or frightening, particularly the deep dissociation of a k-hole.

Recreational ketamine use carries real and immediate risks. The dissociation and loss of coordination can lead to accidents and injuries, and the profound detachment of a k-hole can be frightening and leave a person unable to move or protect themselves, vulnerable to harm. Ketamine impairs awareness and judgement, and combining it with alcohol or other drugs is dangerous, increasing the risk of dangerous sedation, breathing problems, and accidents. Nausea and vomiting are common, posing a choking risk in someone who is heavily dissociated. Beyond these immediate dangers, regular or heavy recreational use carries serious longer-term risks, including dependence and significant physical harm, which are discussed below. So while recreational ketamine use is common, it is far from safe.

Risks of addiction and harm

While ketamine is not considered as physically addictive as some drugs, such as opioids, it carries a real risk of psychological dependence, tolerance, and compulsive use, particularly with regular or heavy recreational use. Users can develop tolerance, needing more to achieve the same effect, and can come to crave the dissociative experience and use ketamine compulsively despite harm, the hallmarks of addiction. Some heavy users experience cravings and find it difficult to stop. So although the withdrawal is generally less physically dramatic than for some drugs, ketamine addiction is real and can take hold, especially in those using it frequently to escape or for its effects.

Heavy or long-term ketamine use can also cause serious physical harm, most notably to the bladder and urinary system. Regular, heavy use can cause a condition sometimes called ketamine bladder or ketamine-induced cystitis, involving inflammation and damage to the bladder, which can lead to severe, painful urinary symptoms, frequent and urgent urination, pain, and, in severe cases, lasting damage to the bladder requiring surgery. Heavy use is also associated with abdominal pain (sometimes called k-cramps), and with cognitive effects such as memory and concentration problems, as well as mental health effects.
