How sensory ataxia and cerebellar ataxia differ, what causes each, their signs, how they are diagnosed, and how alcohol and drugs can contribute.

Sensory ataxia and cerebellar ataxia are two types of ataxia, a loss of coordination and balance, that arise from different parts of the nervous system. Sensory ataxia is caused by damage to the sensory nerves or spinal cord pathways that tell the brain where the body is in space, so balance and gait worsen markedly when the eyes are closed. Cerebellar ataxia is caused by disease or damage to the cerebellum, the part of the brain that coordinates movement, and typically also causes slurred speech, abnormal eye movements, and coordination problems that closing the eyes does not dramatically worsen. Both can result from genetic conditions, disease, or toxins, including heavy alcohol use, and diagnosis relies on clinical examination and tests.

What ataxia is

Ataxia means a lack of coordination that affects movement, balance, gait, and sometimes speech and eye movements. It is a sign of a problem somewhere in the systems that control movement, not a single disease. Two important types are sensory ataxia and cerebellar ataxia, and telling them apart matters because they point to different causes.

Sensory ataxia

Sensory ataxia results from damage to the sensory pathways, the nerves and spinal cord tracts that carry position information (proprioception) from the body to the brain. When the brain cannot sense where the limbs are, coordination and gait suffer. A hallmark is that balance is much worse in the dark or with the eyes closed, because vision is no longer compensating for the missing sense. Causes include peripheral neuropathy, vitamin deficiencies, and spinal cord disease.

Cerebellar ataxia

Cerebellar ataxia results from disease or damage to the cerebellum, the region of the brain that fine-tunes movement. Because the problem is in the coordinating centre itself, closing the eyes does not dramatically worsen it. Alongside an unsteady, wide-based gait, cerebellar ataxia often causes slurred speech, abnormal eye movements, tremor, and clumsy limb coordination. Causes include stroke, genetic conditions, tumours, and toxins.

How alcohol and drugs contribute

Substance use is a relevant cause of both types. Chronic heavy alcohol use can directly damage the cerebellum, producing cerebellar ataxia, and can also cause the vitamin deficiencies (especially thiamine and B12) that lead to sensory ataxia and neuropathy. Certain drugs and medications, and some toxins, can affect the cerebellum or the nerves as well. This is why a careful history, including alcohol and substance use, is part of assessing ataxia.

Diagnosis

Diagnosis begins with a clinical examination that tests coordination, gait, and how balance changes with the eyes closed, along with a review of symptoms and history. Further tests may include blood tests for vitamin levels, nerve studies, genetic testing, and brain or spinal imaging. Identifying the type of ataxia and its cause guides treatment, which targets the underlying problem where possible.

The bottom line
