Alcoholic hepatitis is an acute inflammatory injury to the liver caused by heavy, prolonged alcohol use, and it can be fatal within weeks if severe. It sits on the spectrum of alcoholic liver disease, between fatty liver and cirrhosis, but unlike those conditions it can develop rapidly and without warning. Severe cases carry a 30-day mortality rate of up to 50%, which makes early recognition and treatment critical. What separates it from other liver conditions is the combination of active heavy drinking, sudden jaundice, and liver enzyme patterns that point specifically to alcohol as the cause.

Most patients I see with alcoholic hepatitis did not know they had it until they turned yellow. By that point, many had been drinking at levels most people would call extreme for years, sometimes decades, but had never had a liver test. The ones who do best are the ones who stop drinking completely on the day they receive the diagnosis and never go back. The ones who continue, even at reduced levels, almost always deteriorate.

What Is Alcoholic Hepatitis?

Alcoholic hepatitis is acute liver inflammation triggered by alcohol. The word hepatitis simply means liver inflammation, but alcoholic hepatitis is a specific clinical syndrome with its own diagnostic criteria, scoring tools, and treatment protocols.

It belongs to a broader category called alcoholic liver disease, which runs along a spectrum. Fatty liver, or hepatic steatosis, is the earliest and most reversible stage. Alcoholic hepatitis sits in the middle. Cirrhosis is the end stage, where scar tissue replaces healthy liver cells permanently. What makes alcoholic hepatitis unusual is that it can develop quickly, sometimes within weeks of a particularly heavy drinking period, and up to 20% of cases occur in people without pre-existing cirrhosis.

The mechanism starts with your liver’s attempt to break down alcohol. Your liver uses an enzyme called alcohol dehydrogenase (ADH) to convert ethanol into acetaldehyde, a toxic compound. Acetaldehyde is then converted by aldehyde dehydrogenase 2 (ALDH2) into acetate. When you drink heavily for years, this process overwhelms the liver. Acetaldehyde accumulates, triggering inflammation, oxidative stress, and activation of stellate cells that lay down collagen, which becomes scar tissue.

A secondary pathway kicks in with sustained heavy drinking: the microsomal ethanol oxidising system, which uses an enzyme called CYP2E1. This pathway generates reactive oxygen species that damage liver cells directly. The combination of acetaldehyde toxicity and oxidative stress from CYP2E1 is what drives the acute injury in alcoholic hepatitis.

What Causes Alcoholic Hepatitis?

The primary cause is sustained heavy alcohol consumption. But quantity alone does not tell the whole story.

Most clinical definitions require drinking more than 40 grams of alcohol per day for women and more than 60 grams per day for men for a period of years. That is roughly 3 to 4 standard drinks per day for women and 4 to 6 for men. But some patients develop alcoholic hepatitis at lower thresholds, especially women, because oestrogen appears to increase intestinal permeability to bacterial endotoxins, which amplifies liver inflammation.

Several factors increase your risk beyond drinking volume alone. Genetic variants matter significantly. People who carry the PNPLA3 I148M variant have substantially higher rates of progression to severe liver disease. The ALDH22 variant, more common in East Asian populations, reduces acetaldehyde clearance and increases cellular damage per drink. Obesity increases the risk independently. Hepatitis C co-infection doubles the rate of disease progression.

Malnutrition is both a cause and a consequence. Heavy drinkers typically replace dietary calories with alcohol, leading to deficiencies in B vitamins, zinc, and folate that impair the liver’s ability to repair itself.

Signs and Symptoms of Alcoholic Hepatitis

Jaundice is the defining sign: yellowing of the skin and whites of the eyes. It appears because a damaged liver cannot process bilirubin, the yellow pigment released when red blood cells break down. Bilirubin then accumulates in the blood and deposits in the skin.

Beyond jaundice, the clinical picture typically includes right upper abdominal pain or tenderness, nausea, vomiting, fever (often low grade, around 38°C), loss of appetite, and fatigue. Some patients also present with confusion or altered mental status, which signals hepatic encephalopathy, a serious complication where toxins normally cleared by the liver accumulate in the brain.

In severe cases, you may see signs of decompensated liver function: ascites (fluid accumulating in the abdomen), gastrointestinal bleeding from oesophageal varices, and hepatorenal syndrome, where kidney function deteriorates because the liver is no longer maintaining adequate circulation.

How Alcoholic Hepatitis Is Diagnosed

Diagnosis is primarily clinical. The National Institute on Alcohol Abuse and Alcoholism (NIAAA) consensus definition requires all four of the following: onset of jaundice within 8 weeks, a history of heavy alcohol use for more than 6 months with less than 8 weeks of abstinence before symptom onset, serum bilirubin above 3 mg/dL, and liver enzyme elevations with AST (aspartate aminotransferase) greater than 50 IU/L and less than 400 IU/L, with an AST to ALT (alanine aminotransferase) ratio of at least 1.5.

That AST:ALT ratio is one of the most useful clinical clues. In most liver diseases, ALT rises higher than AST. In alcoholic hepatitis, the pattern reverses. The AST:ALT ratio of 2:1 or greater is a strong indicator that alcohol is the cause.

Lab work also typically shows elevated gamma-glutamyl transferase (GGT), elevated alkaline phosphatase, prolonged prothrombin time (PT) or elevated INR, and low albumin, all reflecting impaired synthetic function. A complete blood count often shows leukocytosis and anaemia.

Liver biopsy is the gold standard for confirmation but is not always necessary or safe. When biopsy is done, it shows hepatocyte ballooning, neutrophil infiltration, and Mallory-Denk bodies (protein aggregates inside damaged liver cells). In practice, biopsy is reserved for cases where the diagnosis is genuinely uncertain.

Prognostic Scoring Tools for Alcoholic Hepatitis

Three scoring tools guide clinical decision-making. Every clinician treating this condition should know all three.

The Maddrey Discriminant Function (MDF) is the oldest and still widely used. The formula is: MDF = 4.6 × (patient’s PT minus control PT in seconds) + serum bilirubin in µmol/L ÷ 17.1. An MDF of 32 or greater defines severe alcoholic hepatitis and indicates a 30-day mortality risk above 30% without treatment. This is the threshold at which corticosteroids are considered.

The Model for End-Stage Liver Disease (MELD) score uses serum bilirubin, creatinine, and INR. A MELD score above 20 in alcoholic hepatitis correlates with significant short-term mortality. A MELD above 25 at 90 days is one of several criteria used when considering early liver transplantation.

The Lille Score is calculated on day 7 of corticosteroid treatment to assess whether the patient is responding. It incorporates age, albumin, bilirubin at baseline and day 7, prothrombin time, and creatinine. A Lille score above 0.45 indicates poor corticosteroid response and predicts high mortality. When the Lille score is above 0.45 at day 7, corticosteroids should be stopped, because continuing them offers no benefit and increases infection risk.

Treatment of Alcoholic Hepatitis

Alcohol Cessation

Stop drinking. This is not a lifestyle recommendation, it is the single most important medical intervention. The liver cannot begin to repair itself while alcohol is still being consumed. Complete abstinence from the moment of diagnosis is non-negotiable. Even a single drink can trigger a new inflammatory cascade on an already-injured liver.

Nutritional Support

Malnutrition is present in nearly all patients with alcoholic hepatitis. Treatment guidelines from the European Association for the Study of the Liver (EASL) and the American Association for the Study of Liver Diseases (AASLD) recommend 35 to 40 kilocalories per kilogram of body weight per day, with 1.2 to 1.5 grams of protein per kilogram per day. In practice, many patients cannot tolerate oral intake and require enteral feeding via nasogastric tube. Protein should not be restricted out of concern for encephalopathy, that practice was abandoned years ago.

Corticosteroids for Severe Alcoholic Hepatitis

Prednisolone (not prednisone, prednisolone does not require conversion by the liver and is the correct choice) at 40 mg per day for 28 days is the standard treatment for severe alcoholic hepatitis defined by an MDF of 32 or greater. The evidence base includes the STOPAH trial (Steroids or Pentoxifylline for Alcoholic Hepatitis), the largest trial conducted in this condition, published in the New England Journal of Medicine in 2015. STOPAH found that prednisolone reduced 28-day mortality but this benefit was not sustained at 90 days or one year, largely because patients who continued drinking deteriorated.

Corticosteroids are contraindicated if the patient has active infection, gastrointestinal bleeding, renal failure, or a positive hepatitis B surface antigen. The decision to start them requires a clear picture of the infection status, which is why initial workup includes blood, urine, and peritoneal fluid cultures.

What Does Not Work

Pentoxifylline was widely used for years as an alternative to corticosteroids or combination therapy. STOPAH effectively ended that practice. Pentoxifylline showed no mortality benefit compared to placebo. It should not be prescribed for alcoholic hepatitis outside a research setting.

Emerging and Investigational Treatments

Granulocyte colony-stimulating factor (G-CSF) has shown promise in several trials by stimulating liver regeneration through mobilisation of bone marrow-derived stem cells. The ATTIR trial and several Indian studies found improved 90-day survival in patients who received G-CSF. This is not yet standard of care outside specialist centres, but it is a legitimate clinical option in severe cases not responding to corticosteroids.

Faecal microbiota transplantation (FMT) is under active investigation based on the established role of gut dysbiosis and increased intestinal permeability in alcoholic hepatitis. Early data are encouraging. Not yet routine practice.

Tip:

If prednisolone is started, always check a Lille Score on day 7. If the score is above 0.45, stopping corticosteroids at that point saves the patient from unnecessary immunosuppression without any mortality benefit.

Liver Transplantation for Alcoholic Hepatitis

For decades, most transplant programmes required six months of documented abstinence before listing a patient with alcohol-related liver disease. The rationale was that six months of abstinence allowed some liver recovery, and demonstrated commitment to sobriety. The evidence base for this specific threshold has always been weak.

Early liver transplantation (before six months of abstinence) is now offered at selected centres for highly selected patients with severe alcoholic hepatitis who have not responded to medical therapy. Criteria typically include a first liver decompensation event, strong social support, absence of other substance use disorders, and commitment from the patient and family. Several European studies, including work from the Paris group, showed survival rates comparable to patients transplanted for other indications, with alcohol relapse rates lower than critics predicted.

MELD score above 25 at 90 days, absence of response to corticosteroids (Lille > 0.45), and MDF above 32 are typical thresholds used when considering early transplant referral.

Complications of Alcoholic Hepatitis

The liver does far more than filter toxins. When it fails, multiple organ systems follow.

Spontaneous bacterial peritonitis (SBP) occurs when bacteria infect the ascitic fluid. It carries a 30% in-hospital mortality rate and requires immediate antibiotic treatment, usually with intravenous cefotaxime. Hepatorenal syndrome (HRS) is a form of acute kidney injury that develops because portal hypertension redirects blood away from the kidneys. It carries extremely high mortality without liver transplantation. Hepatic encephalopathy ranges from mild confusion to deep coma and is managed with lactulose and, increasingly, rifaximin. Oesophageal variceal bleeding requires urgent endoscopic banding or vasoactive drugs such as terlipressin.

Long-Term Prognosis

Prognosis depends almost entirely on two things: whether the patient stops drinking permanently, and whether cirrhosis is already present.

Patients who achieve sustained abstinence after a first episode of alcoholic hepatitis and do not have cirrhosis have a 5-year survival rate above 70%. Patients who continue to drink after a severe episode have a 5-year survival rate under 30%. Underlying cirrhosis worsens the prognosis at every level of drinking.

The presence of PNPLA3 I148M genotype, ongoing hepatitis C infection, and obesity each independently worsen long-term outcomes. These are not modifiable in the short term, but they should inform how aggressively abstinence support is pursued.

When Drinking Has Become More Than Occasional

The pattern I see most often is not a sudden decision to drink heavily. It is a gradual escalation over years, sometimes decades, that neither the patient nor their family recognised as a clinical problem until the liver gave out. By the DSM-5 criteria for alcohol use disorder, you need at least two of eleven criteria in a twelve-month period, including drinking more than intended, failed attempts to cut down, cravings, and continued use despite physical consequences. Many patients with alcoholic hepatitis meet six or more criteria, which places them at the severe end of the disorder. That level of alcohol use disorder requires structured treatment, not willpower alone.

Nutritional rehabilitation, treatment of complications such as spontaneous bacterial peritonitis and hepatic encephalopathy, and active management of alcohol use disorder through structured treatment programmes are all components of care, not afterthoughts. The liver can recover more than most people expect, but only if alcohol is removed entirely and kept out.
