How Alcohol Withdrawal Actually Works

What the Clinical Evidence Actually Says

They have no prior history of seizures during withdrawal. Their liver function is impaired enough that high-dose benzodiazepines carry real risk. Or they have a history of benzodiazepine misuse that makes prescribing a benzo inadvisable.
Gabapentin is particularly useful in patients with co-occurring anxiety, neuropathic pain, or insomnia, because it addresses those symptoms directly rather than just sedating the patient through them. Patients with active opioid use need extra caution. Gabapentin combined with opioids increases respiratory depression risk substantially. That combination requires inpatient monitoring and should not be managed at home.

Gabapentin Abuse Potential and Honest Risk Assessment
The claim that gabapentin is “non-addictive” is not accurate. It has lower abuse potential than benzodiazepines, but it is not zero. People with histories of substance use disorder, particularly opioid use disorder, misuse gabapentin at rates that are clinically significant. Some regions have rescheduled gabapentin because of this.
The euphoric effect of gabapentin at high doses, particularly when combined with opioids or alcohol, is the driver of misuse. At standard clinical doses for withdrawal management, the risk is much lower. But it is a risk the prescribing clinician should assess, not ignore.
For patients actively using alcohol and being prescribed gabapentin during a supervised taper, the interaction is manageable. Taking gabapentin after drinking alcohol significantly amplifies CNS depression. You should not drink alcohol while taking gabapentin for withdrawal. That is not a technicality, it can cause respiratory depression, falls, and loss of consciousness.
Gabapentin vs Other Non-Benzodiazepine Options
Gabapentin is not the only non-benzodiazepine used in alcohol withdrawal. Carbamazepine, an anticonvulsant that works through sodium channel blockade, has the most evidence after benzodiazepines and gabapentin. It has been used for decades in Europe and has good data for preventing seizures in mild-to-moderate withdrawal.
Valproate (valproic acid) is another anticonvulsant used in some protocols. Pregabalin, a structural analogue of gabapentin that also binds the alpha-2-delta subunit, has growing evidence and may have higher bioavailability, though it also carries higher abuse potential.

Gabapentin’s relatively low sedation burden and absence of respiratory depression risk at standard doses make it more suitable for outpatient detox than benzodiazepines in selected patients. Several published protocols have used gabapentin successfully in outpatient alcohol withdrawal, with daily or alternate-day clinician check-ins.
That said, “outpatient” does not mean “unsupervised.” Someone taking gabapentin for withdrawal should have a sober support person at home, should not be driving, and should have a clear escalation plan if symptoms worsen. Daily CIWA-Ar scoring helps track trajectory. If the score climbs above 15 despite gabapentin, the patient needs to step up to inpatient care.
The broader clinical picture matters too. A patient with moderate alcohol use disorder, intact social support, mild-to-moderate CIWA-Ar scores, and no seizure history is a reasonable outpatient candidate. A patient who lives alone, has had prior complicated withdrawals, and drinks a litre of spirits daily is not, regardless of what medication is being used.
When Alcohol Use Has Become More Than Something to Manage at Home
Alcohol use disorder exists on a spectrum, and most people I see in clinic took longer than they expected to recognise where they sat on it.
