Alcohol also suppresses NMDA glutamate receptors, which drive excitation. Drink heavily for long enough, and the brain compensates: it downregulates GABA-A sensitivity and upregulates NMDA activity to restore balance. The brain recalibrates around a baseline that includes alcohol.

Remove the alcohol and the compensations stay in place. Suddenly you have reduced inhibition and increased excitation with no counterweight. The result is a hyperexcitable central nervous system, and that is AWS.

One point competitors often gloss over: DSM-5 includes a specifier for perceptual disturbances. This applies when hallucinations occur with intact reality testing, meaning the patient knows the experience is not real. That distinction matters clinically. A patient who hears voices and believes they are real is closer to full delirium; a patient who sees shadows and knows they are not there is not yet in delirium but is deteriorating. The ICD-10-CM codes reflect this: F10.239 for withdrawal without perceptual disturbances, F10.232 for withdrawal with perceptual disturbances.

Alcohol withdrawal, by definition, can only occur in the context of moderate or severe alcohol use disorder. You cannot code it alongside a mild AUD. This is not a minor bureaucratic detail, it reflects the biological reality that physiological dependence requires sustained, heavy exposure.

Severity Grading: The CIWA-Ar Scale

The Clinical Institute Withdrawal Assessment for Alcohol, revised (CIWA-Ar) is the standard tool for grading AWS severity. It scores ten items: nausea and vomiting, tremor, paroxysmal sweats, anxiety, agitation, tactile disturbances, auditory disturbances, visual disturbances, headache or fullness in the head, and orientation and clouding of sensorium. Each item is scored 0 to 7 except orientation, which is scored 0 to 4. Maximum score is 67.

Symptom-triggered dosing, giving benzodiazepines only when CIWA-Ar crosses a threshold, uses less medication and shortens treatment duration compared to fixed-dose regimens, according to multiple randomised trials. However, symptom-triggered dosing requires staff who can reliably perform the assessment every one to two hours. In settings where that is not possible, a fixed-dose taper is safer.

When You Cannot Use CIWA-Ar

CIWA-Ar requires the patient to cooperate with questioning. It does not work in patients with severe cognitive impairment, significant psychiatric comorbidity, or delirium. In those cases, clinicians fall back on objective vitals, heart rate above 100, blood pressure above 150/90, temperature above 38°C, and clinical judgement. Some centres use the MINDS (Minnesota Detoxification Scale) in these situations.

The Kindling Effect: Why Each Withdrawal Can Be Worse

The kindling effect is the most clinically important concept in AWS that most patients and families do not know about. Each episode of withdrawal sensitises the brain. NMDA receptor upregulation becomes more pronounced with each successive withdrawal. Seizure threshold lowers. Alcohol Withdrawal Seizures

Withdrawal seizures are generalised tonic-clonic events. They occur most commonly between 24 and 48 hours after the last drink. Most are single and self-limiting, lasting one to three minutes. But roughly 3% of patients who seize will progress to status epilepticus without treatment.

Warning:

A withdrawal seizure is a medical emergency. Call emergency services immediately. Do not attempt to restrain the person. Turn them onto their side to protect the airway. Do not leave them alone. A first seizure in the context of heavy alcohol use must be medically evaluated, do not assume it will not recur.

The first-line treatment for withdrawal seizures is a benzodiazepine, typically intravenous diazepam or lorazepam. Phenytoin does not prevent further withdrawal seizures and is not recommended for this indication. Phenobarbital is used in refractory cases or as an adjunct when benzodiazepine doses become dangerously high.

One thing families often ask: will an anticonvulsant from a previous prescription prevent a seizure? The answer is usually no. Warning:

If someone in alcohol withdrawal develops fever, rapid heart rate above 120 bpm, severe confusion, and heavy sweating together, that is delirium tremens until proven otherwise. This requires immediate emergency medical care. It is not something that can be managed at home or monitored overnight.

Risk factors for DTs include a prior history of DTs, prior withdrawal seizures, longer duration of heavy drinking, older age, concurrent infection or illness, and a CIWA-Ar score above 20 on presentation. The presence of any two of these should prompt inpatient admission regardless of how the patient looks at the time of assessment.

Treatment is aggressive IV benzodiazepine dosing, often requiring doses far higher than clinicians unfamiliar with AWS expect. Lorazepam or diazepam IV are first-line. Thiamine 500mg IV three times daily is given alongside to prevent Wernicke’s encephalopathy, which can be triggered or worsened by the metabolic stress of DTs.

Adjunctive medications used alongside benzodiazepines include carbamazepine (evidence-based for mild to moderate AWS, particularly in outpatient settings in Europe), gabapentin, and valproate. None of these replace benzodiazepines in severe AWS or DTs.
